The 29-year-old struggled to find form at Old Trafford and was denied a permanent move by manager Louis van Gaal.
He only managed to score four goals from a total of 26 league appearances, but Blues boss Jose Mourinho has now given him another chance to prove himself in England.
Falcao reached an agreement with the Premier League champions over a season-long loan deal as Mourinho revealed his intentions to help him get back to his best.
He will be added to the squad as a replacement for Didier Drogba, who has been released by the club following a successful return to the team, picking up a league title.
The Colombia international enjoyed great spells for Atletico Madrid ahead of his £52million move to Monaco back in 2013, scoring 70 goals from a total of 90 appearances in all competitions.
